Before Your Visit
If you have been scheduled for Mohs surgery, surgical excision, or PDT (photodynamic therapy), please download, complete and return to us the medical information form below before your scheduled appointment. If you have any questions, please do not hesitate to call our office at 617-573-3789.

Please arrive 15 minutes before your appointment to ensure that all paperwork is done.
Please call Pre-Registration at 617-573-4200 prior to your first appointment to register as a patient with Mass. Eye and Ear and make sure that the hospital has your correct insurance information.
We are located on the 9th Floor, second suite on the left.
Please make sure that we have a copy of your biopsy report prior to your appointment, if applicable.
For surgical appointments, below is a brief list of general postoperative guidelines. These precautions must be taken to minimize complications from surgery, such as bleeding, infection, discomfort and/or poor cosmetic outcome. If you will not be able to follow any of these post-operative restrictions, please let us know as soon as possible as we may have to reschedule your appointment. If you need to cancel or reschedule, or if you have any questions regarding the above information, please call the office at 617-573-3789.
POST-OPERATIVE RESTRICTIONS AFTER SURGERY:
These may include, depending on the procedure:
- A return visit for wound care and/or suture removal within 1–3 weeks.
- Wearing a bandage at the site for the same amount of time.
- Keeping the area absolutely dry for a certain amount of time
- Avoiding exercise after surgery to optimize healing. This includes going to the gym, bending, lifting, swimming, long walks and heavy chores. In general, you should refrain from strenuous activity for at least two weeks after surgery, depending on the site of surgery.
- Daily wound care.
