Online Appointment Request Process
After you have sent us your request, our scheduling staff will contact you within two business days to schedule your appointment. To request an appointment by telephone during office hours, Monday-Friday, 8:30 a.m. to 5 p.m., please call:

International Patient Online Appointment Request Process
Once we receive your completed request form and a detailed clinical summary or medical report (in English), the information will be reviewed to ensure that an appropriate appointment is scheduled. Following the review, you will be contacted by an International Patient Coordinator in approximately two to three business days.
