Insurance plans are agreements made between you and your insurer, and the Massachusetts Eye and Ear Infirmary cannot ensure that an insurance provider will pay for your care.
While the Infirmary accepts nearly every major health insurance plan, as do the individual members of our medical staff, plan benefits change frequently. It is best to call your insurer prior to your appointment to learn the specifics of your coverage and to be sure that you have met all the requirements stipulated by your specific plan.

Prior to your appointment, our registration staff will validate your insurance information with your insurer and obtain any necessary pre-authorizations. If necessary, we will contact you before your visit to obtain additional registration and eligibility information.
Managed Care Referrals
If you are enrolled in a managed care plan, such as an HMO or POS, you may need a referral from your primary care physician. Please contact your physician for authorization well before your appointment or procedure and ask him/her to issue a referral. Your insurance may not agree to pay your entire bill if your primary care physician does not issue a referral prior to your appointment.
Out of Network Benefits
Some insurance companies have restrictions when services are provided “Out-of-Network”. Your insurance company may require a copay/deductible, when you receive treatment out-of-network. Your insurance company may also deny the service entirely, if you do not have out-of network benefits. Please confirm your benefit level with your insurance company.
Be prepared to pay any insurance copayments at the time of your service.
