International medical graduates must be eligible for an H-1B visa and have completed a US or Canadian ophthalmology residency training program. For self-supported fellows (eg the International Oculoplastics Fellow) the H-1B requirement is waived.
Additionally, applicants must be eligible for a full Commonwealth of Massachusetts medical license, the requirements of which can be viewed on the mass.gov website:

All qualifications must be met to be considered for medical staff appointment. Medical staff approval is contingent upon your satisfactory completion of the hospital credentialing process in accordance with Medical Staff By-Laws.
